IMS Equipment Request Form
  • Reservations are made on a first come, first served basis.
  • All reservations are to be made 24 hours before the equipment is required. If your request is less than 24 hours please call the Service Desk at X2569. Requests for Saturday, Sunday or Monday should be made no later than Thursday afternoon. You are encouraged to make recuring reservations for the entire semester at the beginning of the semester.
  • Equipment will be set up 10 minutes prior to the requested time.
  • If a customer pick up is requested, the customer is responsible for pickup and return of equipment.
  • You will receive confirmation of your reservation by email within 24 hours of sending your online request.
  • Student organizations may reserve equipment through their faculty/staff sponsor. Please have the faculty/staff sponsor submit an IMS Equipment Authorization Form.
  • Students may reserve equipment for academic use through their instructor or academic department. Please have the instructor or academic department submit an IMS Equipment Authorization Form.

Reccuring requests can be made with one submission.

  • Same day and time each week. For a Tuesday and Thursday class, enter T TH with start and end time. A request for every Tuesday and Thursday will be entered for the entire semester.
  • Same time and varied dates. Enter the start and end times in the appropriate field. List dates in the additional comments field. A request will be entered for each day indicated.

Need two slide projectors or camcorders? Enter the item in the equipment 1 and 2 field to receive 2, etc, or indicate in the additional comments the number needed.
